During Term 3 we will be updating our student medical records systems to ensure we have the most recent information on your child's health.
New information forms have been emailed to you. If you have not received this email please check with the school to ensure we have your correct email details, and download the form here.

The new forms help us collect your child's important medical information and the introduction of new systems will mean we will be able to better manage the daily and emergency health needs of your child.
We would appreciate your completing and returning the top part of the form with your child's name, Medicare number and doctor's details even if your child does not have any medical conditions or allergies.

Otherwise, you can hand the form in at Reception either via your child or when you are next at school. Alternatively, you could scan the completed form and return it to killara-h.school@det.nsw.edu.au.
